Postoperative Sensitivity After Composite Restorations: Why It Happens and How to Prevent It

Introduction

Composite resin restorations have become the preferred restorative material for most direct posterior and anterior restorations because of their excellent esthetics, conservative cavity preparation, and continuous improvements in adhesive dentistry. Despite these advances, postoperative sensitivity remains one of the most common reasons patients return shortly after treatment.

A typical scenario involves a patient who reports sharp pain while drinking cold water or discomfort during chewing a few days after placement of a composite restoration. The restoration appears clinically satisfactory, the radiograph shows no obvious pathology, and yet the patient continues to experience symptoms.

For many clinicians, the immediate temptation is to assume that the restoration has failed. However, postoperative sensitivity is rarely caused by a single factor. Instead, it usually results from a combination of biological responses and technical errors occurring during cavity preparation, bonding, restoration placement, or finishing.

Understanding the underlying mechanism is essential because successful management depends on identifying the actual cause rather than simply replacing the restoration.


Understanding Why Sensitivity Occurs

The dental pulp is a highly vascular and richly innervated tissue. Whenever dentin is cut during cavity preparation, thousands of dentinal tubules become exposed. These tubules contain fluid that communicates with the pulp.

According to Brännström’s hydrodynamic theory, thermal or mechanical stimulation causes movement of this fluid within the tubules. This fluid movement activates pulpal nerve endings, producing the characteristic sharp pain experienced by patients.

Composite restorations themselves do not cause sensitivity. Rather, sensitivity develops when restorative procedures allow excessive stimulation of the dentin-pulp complex or compromise the integrity of the adhesive seal.


Polymerisation Shrinkage

Every resin composite undergoes polymerization shrinkage during curing. Although the dimensional change is relatively small, the stresses generated at the tooth-restoration interface can be clinically significant.

When composite contracts, it pulls against the bonded cavity walls. If the shrinkage stress exceeds the bond strength of the adhesive interface, microscopic gaps may develop along the restoration margins.

These gaps permit movement of dentinal fluid and bacterial by-products, leading to thermal sensitivity and, in some cases, postoperative pain.

Modern restorative techniques therefore aim not to eliminate shrinkage – which is impossible – but to reduce its clinical consequences through incremental placement, appropriate curing protocols, and careful cavity design.


Adhesive Technique Matters More Than Many Clinicians Realize

The longevity of a composite restoration depends largely on the quality of the adhesive interface.

Even small deviations from the recommended bonding protocol can significantly reduce bond strength.

Common technical errors include saliva contamination, inadequate solvent evaporation, over-drying etched dentin, insufficient adhesive application, or inadequate light curing.

When the hybrid layer is poorly formed, microscopic leakage occurs between the tooth and restoration. Initially this may present as postoperative sensitivity, but over time it may progress to marginal staining, recurrent caries, or restoration failure.

For this reason, adhesive procedures should never be treated as routine steps. They represent one of the most technique-sensitive phases of restorative dentistry.


Is the Restoration Really the Problem?

Not every patient complaining of sensitivity has an adhesive failure.

One of the most frequently overlooked causes is occlusal interference.

Following placement of a posterior composite restoration, even a slight premature contact can produce discomfort during chewing. Patients often describe the tooth as “feeling high” or painful when biting rather than when exposed to cold.

Because the symptoms may resemble pulpal inflammation, clinicians sometimes replace an otherwise successful restoration when a simple occlusal adjustment would have resolved the problem.

Careful evaluation of both static and dynamic occlusion should therefore be part of every postoperative assessment.


Deep Cavities Carry a Greater Biological Risk

As cavity preparations approach the pulp, the remaining dentin thickness decreases considerably.

Deep dentin contains wider and more numerous dentinal tubules, allowing greater fluid movement and making the pulp more susceptible to irritation.

In these situations, even perfectly executed restorations may produce mild transient sensitivity because of the close relationship between restorative procedures and pulpal tissues.

Current evidence supports preserving pulpal vitality whenever possible through minimally invasive cavity preparation, selective caries removal in appropriate cases, and the use of suitable liners when indicated.


Preventing Postoperative Sensitivity

The best treatment for postoperative sensitivity is prevention.

Successful prevention begins long before the restoration is placed.

Accurate diagnosis, careful case selection, and meticulous isolation all contribute significantly to predictable outcomes.

Rubber dam isolation minimizes contamination during adhesive procedures and improves bond reliability.

Composite should be placed in controlled increments rather than as a single bulk mass whenever appropriate, reducing polymerization stress and improving depth of cure.

Equally important is the performance of a thorough occlusal assessment before dismissing the patient. Many postoperative complaints can be prevented by identifying premature contacts immediately after finishing the restoration.


Managing a Sensitive Composite Restoration

Management should always begin with identifying the underlying cause.

Patients experiencing mild thermal sensitivity without spontaneous pain often require nothing more than reassurance and observation, as symptoms commonly improve during the first few weeks.

If the patient reports pain during biting, occlusion should be carefully reassessed before considering replacement of the restoration.

Persistent sensitivity associated with defective margins, contamination during bonding, or obvious microleakage may require replacement of the restoration using improved adhesive protocols.

However, spontaneous pain, prolonged thermal sensitivity, or signs of irreversible pulpitis indicate pulpal disease rather than restorative sensitivity and require appropriate endodontic assessment.

Replacing restorations without establishing the diagnosis rarely solves the underlying problem.
